Friendship Quilt Square Request Form
To have your own friendship square put on our quilt, fill out the request form below. If you would like to use one of our quilt squares above, place the # of your selection in the form. To submit an
image or have your picture placed on our friendship quilt, please
check the "will submit" (Option 2) on the form and email to IplayUnicorns@houston.rr.com.
Please be sure to include your Iplay Nic with the image and put "Friendship Quilt" in the subject line.
